From OP: United Audio 6003 - Youngstown Ohio USA 1969. Sole output by this (studio) ensemble although Mickey Kalis (aka Michael Joseph Kalis) was active in the music business long before and after, but it wasn't a fulltime job. He supported himself as a carpenter, craneman, military man and later as a restaurant owner. He passed away in November of 2016.
